Directions:

If your child(ren) did not receive an iPad during deployment, please complete steps 1-4.

After you have signed the Students Technology Acceptable Use Agreement and Technology Device Agreement we will set up a device and give it to the school to hand out to students. If numbers 3 and 4 are not complete then you will not receive a device for your child. If you have more that one child, you will need to complete forms for each.

1. Read iPad Policies for Parents 

2. Elementary Technology Acceptable  Use  Agreement Form (must be completed to receive an iPad) (grades K-5)  Read, print, and sign a hard copy of the form and send it to school.

3. Middle and High School Technology Acceptable Use Agreement Form (must be completed to receive an iPad) (grades 6-12). Read, print, and sign a hard copy of the form and send it to school.

BOTH FORMS MUST BE COMPLETED OR A DEVICE WILL NOT ISSUED FOR YOUR CHILD.
